Azara Expands EHR Plug-In Integrations for RAF and SMART on FHIR APIs

New embedded capabilities deliver better workflows and accuracy for Azara DRVS users 

Azara Healthcare, the four-time Best in KLAS provider of population health management and value-based care solutions, announced today that it has expanded its Azara EHR Plug-In capabilities. Integrations of the company’s SMART on FHIR and Risk Adjustment Factor (RAF) functionality can now be embedded directly into provider workflows across multiple EHRs, better equipping care teams with insights needed to treat patients at the point of care, without additional clicks. 

Azara DRVS SMART on FHIR Application Integrations (APIs) are now offered for Epic to enable more transparent, standardized information sharing across healthcare stakeholders. DRVS users can access care gap information, pre-visit planning alerts, open referrals, care management plans and additional patient data efficiently within their EHR workflow. This functionality is already available in athenaOne, NextGen and eClinicalWorks. 

Risk Adjustment Factor (RAF) write back functionality is now available in NextGen and eClinicalWorks, in addition to existing integration with athenaOne. The new offering enables providers to more efficiently identify Medicare/HCC and Medicaid diagnosis coding gaps in real-time during patient visits, speeding accuracy and minimizing downstream challenges. DRVS users can dismiss, defer or directly add diagnosis codes for identified documentation gaps, as well as deliver confirmed diagnoses back to the patient’s chart in one click via the “Add to EHR” button.
