The theme of this year’s Forum is “Community Focus For Collective Impact”. Thirty four West Virginia Community Health Centers serve over 550,000 patients, representing over one third of the Mountain State’s population. We have not only a great responsibility, but a great opportunity to use our network and collective strength to impact the communities we serve in a positive way. This year’s Forum will focus on topics suggested by you- such as updates from state and federal partners, AI in healthcare, value-based forecasting, quality improvement success stories, SDoH, leveraging population health, workforce and culture, and advocacy.
