Azara Care Connect: Revolutionizing Care Coordination

New application offers a user-friendly interface and simplified workflow for outreach documentation

Azara Healthcare, a leading provider of population health solutions, announced today the launch of its new care coordination application: Azara Care Connect. Care Connect draws clinical, visit, and demographic data from the Azara DRVS platform and integrates it with data supplied by the Health Plan—providing outreach staff with timely and robust information to support better informed outreach decisions. 

“Azara Care Connect enables practice staff to organize patient data from health plans, including care gaps, to quickly and easily perform and document outreach and outcomes for each patient,” said Matthew Fusan, Azara’s SVP Product. “This powerful new application provides patient prioritization algorithms, tracking, and reporting of all activities in a centralized application—driving higher gap closure with reduced manual efforts." 

Azara Care Connect updates in real time, allowing users to enter activities and review outreach history continuously. Outreach staff are able to record the reason, method, and outcome of every contact as well as enter notes and set follow-up events. For every completed patient contact, the services and resources provided, assessments administered, and needs identified can all be captured through one workflow.  

“The data coming out of the Care Connect application saves us tremendous time in the way of reporting performance/activity both internally and to our external Health Plan partners, said Chris Hicks, Network Analytics Director at Michigan Community Health Network. “I would say the time savings is 70%, as several of us invested a lot of time each month reviewing, cleaning and normalizing outreach activity data submitted independently by each CHW.”  

With the ability to track activities and complete gaps, Azara Care Connect helps organizations and teams focus on patients most in need of intervention. The completion of a Social Determinants of Health (SDoH) Needs Assessment and the identification of needs can also be recorded, along with any identified reasons for overuse of Emergency Department (ED) services.
