Azara congratulates its clients named 2022 Million Hearts® Champions. Million Hearts® is recognizing 9 solo practitioners, large health systems, community health centers, and specialty groups as 2022 Hypertension Control Champions. These Champions were able to achieve blood pressure control for at least 80% of their adult patients with hypertension.
Two of the nine FQHCs named 2022 Champions are Azara DRVS users:
- Community Health Center of Southeast Kansas, Pittsburg, Kansas*
- Valley Professionals Community Health Center – North Terre Haute, Terre Haute, Indiana*
This year’s Champions join other Azara clients previously recognized by Million Hearts including Lynn Community Health Center in 2018 and La Clínica Tepeyac, Inc., Maple City Health Center, Northeast Missouri Health Council, PrairieStar Health Center, and Valley Professionals Community Health Center (Rockville, Bloomingdale) in 2020.
