Azara's 2022 Annual User Conference has ended. 

Over 345 professionals from 32 states made this year's event a great success! Stay tuned for 2023 dates and information.

DRVS users from Community Health Centers, physician practices, Primary Care Associations, Health Center Controlled Networks, and clinically integrated networks are invited to join us for a day of pre-conference workshops and two days of general sessions, breakouts, and networking events.

Our growing national conference, now back in person, is the ideal opportunity for DRVS users to meet, share best practices and ideas, further their knowledge and skills, and collaborate with the Azara team. Sessions cover topics appropriate for all levels of DRVS users—new to advanced—and aim to broaden and deepen the use of solutions and modules. The 2022 User Conference content is designed to empower attendees to maximize their use of Azara tools to drive improvements in care delivery and positively impact patient lives.

The theme of this year’s conference is Metamorphosis, where the spirit of spring renewal inspires and encourages us to embrace the future of transformative change.

Pre-Conference Workshops:

Pre-conference workshops take place May 2, 2022 from 10:00 AM - 3:30 PM and include lunch plus a workshop attendee reception from 5:00 - 7:00 PM.  Seating is limited. Choose one of three options.

Catalyzing Transformation at the Network Level: Creating an Environment for Change

Join us for a hands-on workshop specifically designed for users looking to drive large-scale, sustainable change with their data at the network or organization level. This workshop caters to more sophisticated DRVS users or those looking to explore the full breadth of what an integrated data platform can do to improve quality, engage patients and reduce provider and staff burden. Participants will have the opportunity to learn about and practice with the unique tools DRVS offers to help promote a culture of data-driven decision making across an entire network or organization, not just with one team or individual. Attendees will have the opportunity to work with other members of their organization as well as users from across the country to hear best practices and share ideas.

Topics that will be covered include:

  • Building a culture of continuous quality improvement with data
  • Customizing and using the right tools for the right role
  • Developing data driven success stories to highlight return on investment
  • Increasing DRVS use across a network or organization

At the end of the workshop, participants will have a greater understanding of how DRVS can be integrated into the ecosystem of a network or large organization. The key takeaway is to have users conceptualize initiatives and goals with data in mind, and subsequently leverage that data across a variety of programs, organizations, or locations.

Driving Success in Value-Based Care: Best Practices & Strategies to Take You to the Next Level

To be successful in the current environment of ever-increasing demands on providers and systems, an organization must be agile and seek to use the data sources available or find ways to get them.  In this interactive workshop we will cover key concepts and actions that will help lead to success regardless of the type of value-based care arrangements you are participating in.

Topics that will be covered include:

  • Identifying high-need patients by leveraging risk stratification tools
  • Effectively managing complex medical conditions by using Azara Care Management to assist patients and their support systems
  • Driving care gap closure by leveraging care coordination tools to cast a broad net across your population
  • Engaging your patients using automated patient outreach and text-based messaging
  • Reducing high-cost emergency department and hospital visits by effectively managing transitions of care
  • Improving outcomes and reducing inefficiencies by partnering and strengthening relationships with your health plans

During this collaborative workshop, we will explore various strategies for your organization to drive success in value-based care and care management models. The workshop will also include use cases from Azara clients that have successfully implemented these strategies and facilitate a panel discussion that all participants can engage in to share best practices and ideas.  This workshop is the catalyst needed to create a clear path to integrated value-based care.
